SUBMIT A CLAIM
CLAIMS PROCESS
Choose from several available claim options, including our Client Zone online self-help portal.
STEP 1
SIGN IN TO OUR CLIENT ZONE, OR DOWNLOAD A CLAIM FORM
Option 1: Register and sign in to your online Client Zone account to submit your claim online. This is the quickest option and submits your claim to us immediately.
Option 2: Download a claim form, or call our Call Centre on 0861 001 788 to assist.
STEP 2
COMPLETE THE CLAIM FORM
Complete the online or downloaded claim form. The claim form needs to be accompanied by the supporting documents detailed in Step 3 below.
STEP 3
GATHER SUPPORTING DOCUMENTS
Please make sure that all of the following supporting documents accompany your claim:
- A clear certified copy of the claimant/beneficiary’s identity document
- A clear certified copy of the deceased’s identity document
- A clear certified copy of the deceased’s death certificate
- A clear copy of the BI1663 form
- Proof of banking details, i.e. bank statement
- Most recent salary slip of the claimant
- Police report. (This applies in the event of unnatural death)
Other documents may be required from the member depending on the nature of the claim possibly including a Marriage Certificate/Decree of Divorce and Letter of Authority, deceased profile.
STEP 4
SUBMIT YOUR CLAIM
Submit your claim together with all the supporting documentation online or by emailing us at claims@mhasa.co.za, by fax at 087 230 1789, or by calling our Call Centre on 0861 001 788.
